BMHA HEALTH CHECK

 

Burnaby Minor Hockey Association requires teams to perform a COVID-19 health screening and collect contact tracing information for all event attendees.

For attendance & health check, use the forms linked below for each facility:

• Bill Copeland Sports Center
• Burnaby Lake Arena
• Kensington Arena
• Scotia Barn (formerly 8-rinks) - BMHA Form and CANLAN Form

IMPORTANT NOTE:   You cannot go back or access the "cleared" health check once the browser has been closed.  In case you need to close your browser, it is recommended you take a screen shot of the "cleared" health check.
