Please ignore reference on the form to multiple copies. You only need to submit one copy.
Note: be sure to include the email address you want BMHA correspondence sent to. Next to our website, this is our primary method of communication.
If you are unable to download an Adobe file, or cannot print it out, copies will be available at our registration session.
4) Payment
Visa and Mastercard credit cards are our only accepted method of payment.
NO CASH OR CHEQUES WILL BE ACCEPTED. While we recognize that this may be difficult for some members, we have had to institute a strict policy due to the unacceptable number of NSF cheques received in past years. Money orders and cash have also proved problematic.
5) BMHA Constitution & Bylaws and Policies
By registering with Burnaby Minor Hockey you are agreeing to all rules and regulations outlined in the association's Constitution & ByLaws and policies as outlined in our Policy Manual. Be sure to review all documents BEFORE registering with the association.
Be aware that you and your child will be required at the start of the season to sign Player and Parent Contracts that outline what is expected of our members.
Burnaby Minor Hockey, like all Minor hockey associations, is a non-profit, membership-driven association who expects their membership to participate in volunteer duties at a team and association level, and to be supportive of all the children and volunteers within the association.

General Info
Contact vs Body-checking Hockey:
When signing up for our General or Integrated Hockey (Boys AND girls league, Peewee, Bantam or Midget), please note that you will have to decide between Bodychecking (Hitting) and Non-Bodychecking (Non-hitting).
To clarify: Hockey 1, 2, 3, 4 and Atom play Non-Bodychecking(Non-Hitting) hockey automatically. At the higher levels, you have a choice between the two for the House leagues. Please be sure to make the RIGHT choice for your child.
Transferring Player
If you are a new player to BMHA, but are transferring from another Association, you will need to provide the above proof of identity and residence as well as a “Player Movement Form” (which can be found at www.burnabyminor.com under the “Forms” tab). It must be completed and signed by the old Association and both parents even if the player only lives with one. There will also be a player movement fee.
Transfer between BMHA and BWC
Player movements between Burnaby Minor (BMHA) and Burnaby Winter Club (BWC) will be allowed up to and including September 1st as per the Memorandum of Understanding between the BWC and BMHA signed March 2009.
